- Grace is a highly organised and conscientious nurse with a strong commitment to delivering high quality, compassionate patient care. As an experienced Advanced Nurse Practitioner with a background in District Nursing and a specialist interest in rehabilitation, geriatric medicine and frailty, she is passionate about understanding what matters most to patients and ensuring care is person-centred, holistic and responsive to individual needs. In 2019, Grace was awarded the prestigious title of ‘Queen’s Nurse’ in recognition of her commitment to high standards of care, lifelong learning and professional development.
- Since November 2019, Grace has worked as an Advanced Nurse Practitioner within Sussex Community NHS Foundation Trust, providing autonomous care for predominantly older frail patients within a community hospital setting. The role involves managing patients requiring inpatient rehabilitation as both step-down from acute services and step-up from the community. Working closely with multidisciplinary colleagues, Grace independently admits and reviews patients, manages acute and chronic conditions, and provides assessment and treatment for illness and injury. Her clinical expertise includes advanced physical assessment, independent prescribing, blood test interpretation, ECG interpretation, wound and pressure area care, catheter and continence management, bowel care, palliative care, pain management and medicines optimisation. Grace is proficient in the management of long-term conditions including diabetes, COPD, heart failure and hypertension. Alongside direct patient care, she coordinates complex discharge planning, advanced care planning and ReSPECT discussions, while also leading on frailty service development across the Trust. Grace is passionate about staff development and supporting staff education, actively contributes to advanced practice policy development and acts as a practice supervisor and assessor for postgraduate nursing students.
- Prior to this, Grace worked within community nursing services in a range of leadership and clinical roles, including District Nurse, Clinical Nurse Lead, Community Sister and Community Staff Nurse. These positions developed her expertise in holistic assessment, care planning, chronic disease management, leg ulcer care, compression therapy, palliative and end of life care, and leadership within multidisciplinary teams. Grace has extensive experience coordinating community teams, triaging referrals, allocating workload, supporting staff development and managing appraisals and supervision.
I have over 17 years' nursing experience across both hospital and community healthcare settings.
Passionate about providing patient-centred holistic care and maintaining and supporting high standards in clinical practice, my career to date has been rich in learning and development opportunities. Sharing this learning through supporting staff and post-graduate students in clinical practice and through quality improvement work has further embedded lifelong learning into my nursing practice.
With a focus on rehabilitation and complex case management I have always enjoyed being able to make a difference to patients and their families/carers.
