Coroner Reports
At TG Associates we understand that a coroner may need the guidance of an expert witness to assist in providing a clear and concise evaluation of events when it has not been possible to establish answers from the evidence already presented to them.
Our reports focus on the systems of working in place, and any failings in those systems, strictly insofar as they caused or meaningfully contributed to a death.
Our expert reports will:
- Document any failings in relation to the standard of care in the lead up to the death.
- Consider if there were any defects, in any system of working, or the absence of any system of working, which caused or materially contributed to a death.
- Whether a death may have been prevented by way of any precautions which could reasonably have been taken, and if they had been taken, might realistically have resulted in the death being avoided.
- Identify any defects in any system of work, and/or any reasonable precautions which could have been taken and, if so, comment on whether these relate to matters which are likely to be of an ongoing concern or present an ongoing risk.
- Document other information or observations which we believe may be relevant to the investigation.
